Club Insurance
All Ontario Soccer clubs can receive insurance coverage for their events and programming. Clubs must receive insurance coverage for each location they use. For example, if a house league program takes place at Location A and competitive matches are played at Location B, both locations A and B will need a separate Certificate of Insurance Request Form submitted.
Please fill out the Certificate of Insurance Request Form below and submit to hdsa@hdsasoccer.ca. Policies are usually completed within 48 hours. Please note that this policy is renewed in April each year and you will have to renew each certificate for the new season.

During peak times during the season, the office staff receives a high volume of emails and calls. If something is urgent - email is the best way to get a hold of us as the voicemail is monitored less frequently.
